Vancomycin Dosing for Dogs, Cats, Horses By Weight
Important Safety Information
Vancomycin should be administered as an IV infusion over at least 1 hour to reduce the risk of infusion-related reactions.
Monitor renal function and serum concentrations during therapy. Use with caution in patients with renal impairment.

Additional Information
Dogs: Constant-rate infusion (CRI): Loading dose of 3.5 mg/kg, followed by CRI of 1.5 mg/kg/hr mixed in 5% dextrose in water.
